Information and declaration

Dear Sirs,

I would like to inform you that as of 28 February this year, the agreement with Nationale Nederlanden concerning the voluntary group insurance for employees of our University will expire.

Therefore, in order to ensure the continuation of the voluntary insurance cover for employees, the university has signed a proposal to take out a policy with the insurance company WARTA from 1 March 2019, for a period of one year with the possibility of extension.

The monthly contribution is 46.00 PLN or 57.00 PLN per person depending on the option chosen.

Employees who wish to be covered by such insurance are asked to complete a declaration to this effect, a form of which is available in the annex, together with other information on the offer and the terms and conditions of the insurance.

The completed document must be submitted in hard copy to the Payroll Department by 15 March 2019.

Employees joining the group insurance are also required to submit a declaration of consent to payroll deduction of contributions - can be found in the annex.
